This is a short introductory conversation about your therapeutic needs (individual or couples/marriage) as well as an opportunity for me to provide you with information about my practice, location, pricing, and therapy practices.

What is an Intake Appointment?

The intake appointment is a unique opportunity for me to learn about you, your life, and what you are seeking counseling services for. This 90 minute session is the time where we will talk about and explore your unique lived experiences (development, relationships, emotions, behaviors, family etc.) as well as map out goals for the therapy process. This opportunity is also the time where you are able to ask me questions about myself, my training, my education, as well as explore my office and the unique structure of my office (interacting with the library of books on display, the art bins, sensory items, sand tray Zen garden, meditation/breathing exercises). Learning about you is just as important as you learning about me. We must feel comfortable, safe, and secure to truly engage in the authentic therapeutic journey.

If you would prefer to contact me directly, please use the "Contact Information" page where you can find my clickable email link and a hyperlink to my phone number.

When emailing me, please provide me with as much information as necessary to ensure that I am able to contact you directly, through phone or through email correspondence. 

When contacting me through a telephone call, please leave a detailed voicemail message with your name, phone number, and a brief explanation of your service needs. I try and return all voicemail messages and emails within 24 hours.


Individual Therapy

I have found that integration of clients passions into the therapy work helps clients to gain a more authentic connection to themselves and the therapy process. I truly believe in meeting clients where they are, understanding clients from a whole person perspective and lens, and walking beside them as they navigate and learn about themselves, emotions, relationships, and the world. 

Individual counseling services will use one on one sessions to focus on exploring lifespan, relationships, and self from a bio-psycho-social and psychodynamic approach while using art, music, educational resources, and mindfulness modalities and interventions. I specialize in working with:

Couples and Marriage Therapy

Working with couples allows for integration of an integrative and eclectic mode of therapy where individual lifespan and couples experiences are explored and forged. Using The Gottman Method along with Dr. Alexandra H. Soloman's Relationship Work to explore the interpersonal and intra-psyche relationship challenges and build individual awareness of the unique lifespan influences of one another along with learning about the development of the self. This self fosters and encourages an authentic awareness of the self and of your partner which allows for and encourages compassionate and empathic relational growth. 

Whether the couple is needing support to effectively communicate, reignite passion/intimacy, work through the challenges associated with continuing with the relationship or take the next steps in their relationship, I willingly participate, facilitate, challenge, encourage, and support the couple without bias or preconceived judgement to help them connect, learn, and grow together.

Integrative Therapy Models Used

Collaborative and Integrative Approaches are used to ensure Clients receive the care and support they need throughout their therapy process:

Intake Paperwork and Consent Forms

All Intake Paperwork and Consent Forms are provided to clients electronically through the Patient Portal. The patient portal is a HIPAA compliant and a confidential platform that is used for all client demographic, consent forms, scheduling, billing, and counseling session documentation services. Each client has their own unique credentials for logging in and for viewing upcoming appointments as well requesting appointments. This is also the place where you will complete and submit Client Information Forms and Consent Forms. 

During our Phone Consultation, your secure and individualized client profile will be created and an electronic link will be sent to the email address you provide. This link will grant you access to your profile so that you can complete all documents and personal information.

Consent forms are required for all counseling services as it grants me permission to work with you and you are agreeing to work me! 

Should you ever forget or lose your password for the portal, send me a quick email message requesting a password reset or use the "Password Reset" button on your "log-in" page and an email will be sent to you to reset your password.

Why Are My Services Cash Pay?

Being able to choose the person whom you receive counseling services from is important- one must trust the person to trust the process. As if you are choosing a doctor or other medical professional, it is important that you are able to make decisions for your own well being without being tied down or restricted. While using insurance is a great financial supplement, insurance requires a medical diagnosis, which can be a scary and an overwhelming process. When it comes to quality of care and autonomy in medical decision making for self, you should not be restricted to the limitations of insurance. Prioritizing your mental health is the equivalent to prioritizing your physical and medical health. 

What to check to see if your insurance provider will reimburse you for Out-of-Network services? Contact your insurance provider directly and discuss the options that they have for your to receive possible Out-of-Network reimbursement. Following each counseling session, a Super Bill document can be provided to you to submit to your insurance company. While there is no guarantee of reimbursement, it never hurts to ask this information and to be prepared in advance for the options you have for counseling autonomy and agency over your mental well-being.

Privacy Policy and Protected Health Information Disclosure

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

[Anchored Within Counseling, LLC] (the “Practice”) is committed to protecting your privacy. The Practice is required by federal law to maintain the privacy of Protected Health Information (“PHI”), which is information that identifies or could be used to identify you. The Practice is required to provide you with this Notice of Privacy Practices (this “Notice”), which explains the Practice's legal duties and privacy practices and your rights regarding PHI that we collect and maintain.

YOUR RIGHTS

Your rights regarding PHI are explained below. To exercise these rights, please submit a written request to the Practice at the address noted below.

To inspect and copy PHI.

  • You can ask for an electronic or paper copy of PHI. The Practice may charge you a reasonable fee.

  • The Practice may deny your request if it believes the disclosure will endanger your life or another person's life. You may have a right to have this decision reviewed.

To amend PHI.

  • You can ask to correct PHI you believe is incorrect or incomplete. The Practice may require you to make your request in writing and provide a reason for the request.

  • The Practice may deny your request. The Practice will send a written explanation for the denial and allow you to submit a written statement of disagreement.

To request confidential communications.

  • You can ask the Practice to contact you in a specific way. The Practice will say “yes” to all reasonable requests.

To limit what is used or shared.

  • You can ask the Practice not to use or share PHI for treatment, payment, or business operations. The Practice is not required to agree if it would affect your care.

  • If you pay for a service or health care item out-of-pocket in full, you can ask the Practice not to share PHI with your health insurer.

  • You can ask for the Practice not to share your PHI with family members or friends by stating the specific restriction requested and to whom you want the restriction to apply.

To obtain a list of those with whom your PHI has been shared.

  • You can ask for a list, called an accounting, of the times your health information has been shared.  You can receive one accounting every 12 months at no charge, but you may be charged a reasonable fee if you ask for one more frequently.

To receive a copy of this Notice.

  • You can ask for a paper copy of this Notice, even if you agreed to receive the Notice electronically.

To choose someone to act for you.

  • If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights.

To file a complaint if you feel your rights are violated.

  • You can file a complaint by contacting the Practice using the following information:

  • The Practice will not retaliate against you for filing a complaint.

To opt out of receiving fundraising communications.

  • The Practice may contact you for fundraising efforts, but you can ask not to be contacted again.

OUR USES AND DISCLOSURES

1. Routine Uses and Disclosures of PHI

The Practice is permitted under federal law to use and disclose PHI, without your written authorization, for certain routine uses and disclosures, such as those made for treatment, payment, and the operation of our business. The Practice typically uses or shares your health information in the following ways:

To treat you.

  • The Practice can use and share PHI with other professionals who are treating you.

  • Example: Your primary care doctor asks about your mental health treatment.

To run the health care operations.

  • The Practice can use and share PHI to run the business, improve your care, and contact you.

  • Example: The Practice uses PHI to send you appointment reminders if you choose.

To bill for your services.

  • The Practice can use and share PHI to bill and get payment from health plans or other entities.

  • Example: The Practice gives PHI to your health insurance plan so it will pay for your services.

2. Uses and Disclosures of PHI That May Be Made Without Your Authorization or Opportunity to Object

The Practice may use or disclose PHI without your authorization or an opportunity for you to object, including:

To help with public health and safety issues

  • Public health: To prevent the spread of disease, assist in product recalls, and report adverse reactions to medication.

  • Required by the Secretary of Health and Human Services: We may be required to disclose your PHI to the Secretary of Health and Human Services to investigate or determine our compliance with the requirements of the final rule on Standards for Privacy of Individually Identifiable Health Information.

  • Health oversight: For audits, investigations, and inspections by government agencies that oversee the health care system, government benefit programs, other government regulatory programs, and civil rights laws.

  • Serious threat to health or safety: To prevent a serious and imminent threat.

  • Abuse or Neglect: To report abuse, neglect, or domestic violence.

To comply with law, law enforcement, or other government requests

  • Required by law: If required by federal, state or local law.

  • Judicial and administrative proceedings:  To respond to a court order, subpoena, or discovery request.

  • Law enforcement: For law locate and identify you or disclose information about a victim of a crime.

  • Specialized Government Functions:  For military or national security concerns, including intelligence, protective services for heads of state, or your security clearance.

  • National security and intelligence activities: For intelligence, counterintelligence, protection of the President, other authorized persons or foreign heads of state, for purpose of determining your own security clearance and other national security activities authorized by law.

  • Workers' Compensation:  To comply with workers' compensation laws or support claims.

To comply with other requests

  • Coroners and Funeral Directors: To perform their legally authorized duties.

  • Organ Donation: For organ donation or transplantation.

  • Research: For research that has been approved by an institutional review board.

  • Inmates:  The Practice created or received your PHI in the course of providing care.

  • Business Associates: To organizations that perform functions, activities or services on our behalf.

3. Uses and Disclosures of PHI That May Be Made With Your Authorization or Opportunity to Object

Unless you object, the Practice may disclose PHI:

To your family, friends, or others if PHI directly relates to that person's involvement in your care.

If it is in your best interest because you are unable to state your preference.

4. Uses and Disclosures of PHI Based Upon Your Written Authorization

The Practice must obtain your written authorization to use and/or disclose PHI for the following purposes:

Marketing, sale of PHI, and psychotherapy notes.


You may revoke your authorization, at any time, by contacting the Practice in writing, using the information above. The Practice will not use or share PHI other than as described in Notice unless you give your permission in writing.

Good Faith Estimate- No Surprise Fee's

To Ensure transparency for the fee's associated with services rendered within Anchored Within Counseling, LLC here is a breakdown of the fee's that each client is subjected too and responsible for:

Session Fee's

Intake/First Initial Session: $270 (one time fee at the start of therapy)

Individual Counseling: $135 (per session)

Family Counseling: $180 (per session)

Couples/Marriage Counseling: $180 (per session)

Add on Half Hour Session Fee:

Individual Counseling: $67.50

Family Counseling: $90.00

Couples/Marriage Counseling: $90.00

Cancellation Fee:

$270 for Intake Session 

$135 Individual Counseling Session

$180 for Couple/Family Counseling Session

Records Request:

Records request must be made in writing and must state the exact type of record being requested (complete client file or statement of services). 

Please allow 7-10 business day's for records requests to be completed.

Complete Client File- 1 (one) printed copies of all medical records which includes- Intake Document, Consent Forms, Diagnosis information, Treatment Plan, Session Notes, Completed Assessments/Questionnaire Outcomes, Payment Statement- $150 per hour

Statement of Services- 1 (one) printed copy- Brief Statement of Services Received, Type of Therapy being Received, Diagnosis Codes and Definition, Outcome Measures, and Discharge Synopsis- $150 per hour

*ALL records requests must be picked up directly from myself during regular business hours. ALL records request MUST be signed for upon receipt. Records request MUST be picked up by the client or clients guardian (proof of ID will be required)*
